Mediation, health literacy, co-construction, commitment: the Haute Autorité de Santé's avenues for "making digital a lever for improving the quality of care"
In a , "Numérique: quelle (R)évolution?", released in June 2019, the French National Authority for Health (HAS) reviews the main issues of digital for the fields of health and social support. How can all the players be mobilized? What uses should be encouraged? How can digital technology be used to improve the quality of care and support? What evaluations should be put in place to measure quality and develop trust?report ofForward-looking analysis
Among the conditions and priorities, the HAS pointed, first of all, to the need for progress in digital mediation.
"Digital mediation for all, because it is not only about targeting the most precarious or vulnerable audiences, but also all those who have a technical or cultural difficulty with digital technology. Especially since the tools of health literacy or digital literacy could potentiate their efforts to reduce this cultural gap. In addition, if digital solutions were co-constructed with their final beneficiaries, the need for mediation would be further reduced.
The HAS proposed to "promote digital mediation, through a repository of skills, training and good practices, without prejudging the professions, functions and places in a position to provide this digital mediation" .
"Expectations go beyond just people in precarious situations," observes the HAS. "They concern many other groups, sometimes simply ill-equipped with digital culture and tools. The elderly are often mentioned, but it is the whole of society that lives in the digital age and may need support to familiarize themselves with its tools, at least for their information and awareness of the risks before it is for their care or social benefits.
The HAS also pointed out in its report the importance of using digital technology to improve the quality of care and support.
"In order to face the current challenges of organizing the pathways of users of thehealth and social services, the structural transformations to be undertaken are major andmany of them can benefit from digital solutions. It facilitates the daily work of professionals, the coorand teamwork, as well as the relevance and continuity of their work.care and accompaniment. Since 2020, the French health insurance system has been deploying digital inclusion initiatives
The identification and tracking of insured persons who are far from digital is carried out by the partners of the Assurance Maladie or by the Assurance Maladie itself (in physical or telephone reception for example).
Different situations exist:
beginners who have no experience of digital technology: they should be referred to a partner digital mediation structure, in order to benefit from reinforced digital support;
Intermediary policyholders who already have an e-mail address but are not comfortable with computers and online procedures: it is possible to offer them individual or group digital workshops conducted by their health insurance fund.
When a partner identifies a digitally remote insured:
he/she can refer the matter to the Mission accompagnement santé of the health insurance fund of the insured person's residence, using the referral form ;
it can promote these digital inclusion workshops to the insured and show them how to contact their health insurance fund themselves.
During the workshops, the health insurance fund provides insureds with digital devices (laptops, tablets, smartphones); insureds can also bring and use their own equipment, if they wish.

My Health Space: the implementation of a new public service
Tested since July 2021 in three pilot departments, Haute-Garonne, Loire-Atlantique and Somme, the digital health space is open to all users since February 3, 2022.
The digital health space is intended to facilitate user procedures and exchanges with health professionals for better care, and will replace the Shared Medical Record (DMP).
The digital health space, called My Health Spacealready includes :
an improved version of the DMP to store and share all health data: prescriptions, treatments, test results, medical images, medical history and allergies, hospitalization reports, vaccinations, etc;
a secure messaging system for exchanges between patients and healthcare professionals.
In a second stage, this personal digital space will be completed with two new services:
a health diary to manage medical appointments and receive reminders for key dates of check-ups (check-ups, mammograms, vaccinations, etc.);
a catalog of state-certified applications to help you choose the digital tools you need to monitor your health.
From January to March 2022, Medicare plans to send more than one million activation codes to insured persons each day by email or, failing that, by mail.
To open an account, you will need this code, your health insurance card and a computer connected to the Internet to connect to the site MonEspaceSanté.

The network of health insurance funds will be able to answer questions from insured persons about activating their health space and direct them to support services.
More broadly, digital mediation actors will be trained in My Health Space to be able to support citizens who need it.
In addition, all individuals and volunteer organizations willing to help their fellow citizens use My Health Space will be federated by regional coordinators recruited for the occasion by the Regional Health Agencies (ARS) and the regional e-health development support groups (GRADeS).

A pilot phase of experimentation
Insureds in three departments - Haute-Garonne, Loire-Atlantique and Somme - were able to try out the service.
The objective of this pilot phase was to technically test the service on a real scale in order to improve it before its generalization.
At the end of October, Mon espace santé was open to 3.3 million people residing in these departments. Less than 1% (0.7%) of insured persons were opposed to its opening.
This first stage of the deployment allowed for technical testing of the platform's operation, and in particular its articulation with the software used by healthcare professionals and institutions in the field. User tests were conducted with pilot users and patient associations.
To observe the use of the service, health professionals and institutions participated as beta testers in this first stage.
Their feedback shows, firstly, that the automatic creation of My Health Space has brought about a significant change of scale for healthcare professionals who, no longer wondering whether their patient has a DMP, are now making a habit of adding to it whenever necessary.
The tests also enabled the service to be enriched in terms of sections, to test its ergonomics, to improve the messaging system (management of recipients, sharing of medical profiles and documents per message), and to refine the way in which the DMP and My Health Space are linked.
Also deployed on a large scale, health messaging between healthcare professionals (in cities or institutions) and patients has made it possible to test new uses, such as sending exam invitations, prescription renewals and care plans.

A citizen's committee on digital health
In order to involve citizens in the construction of this new public service, the Ministry of Solidarity and Health had implemented a participatory approach:
Citizen Workshops, in 2019, with about fifty citizens, with profiles representative of French society, and then Citizen Conferences, in 2020;
A Citizen Committee, composed of 28 members representative of the general population, met several times in 2021, to answer some key questions on data security and digital sovereignty in health, the right to access health data or digital inclusion.
"More than 60% of citizens' contributions have been directly taken into account in the construction of My Health Space. The rest of the proposals are being processed. It sets a citizen framework for future developments of the service."
